Social Prescribing Link Worker (Swaffham and Downham PCN), Norfolk Services – Based in Swaffham

£24,004 – £26,439 FTE (£14,597.03 – £16,077.77 per annum, pro rata for corresponding part-time hours)

Minimum 22.5 hours per week (part-time) – additional hours, up to full-time (37 hours per week), will be considered, please state your preference on the application form. Will include some evenings and some Saturday mornings.

Temporary Contract: initially until March 2024 with possibility of extension.
