Families Together Leeds

Family Action Families Together Leeds service is commissioned by Leeds City Council to provide family support for children and families across Leeds.

Referrals are accepted into the service via Leeds City Council’s family support allocations panel.

Our Family Support Workers:

• work with families across the city of Leeds
• talk with families to identify and assess their needs, often building on assessments by other partner agencies
• work with families and other partner agencies to develop a plan to address families’ needs, with clear and realistic outcomes
• meet with families regularly to review outcomes to support continued progress for the family
• work with families to develop support networks in their local community
