Families Together
Supporting families in Wales during the perinatal period, with mental health, and/or if they are experiencing domestic violence
The Families Together Project provides support to families facing multiple and complex needs including those living with mental ill health and/or experiencing domestic abuse, with a particular focus on supporting families in the perinatal period (the period leading up to and following the birth of a baby).
The project supports families to overcome the challenges they face through tailored support for all family members, increasing their confidence, wellbeing and resilience, ensuring the safety of all family members and developing their capacity to plan for the future. Working in partnership with other local agencies we identify issues around mental health, domestic abuse and family conflict early, and deliver tailored support to prevent issues escalating.

Mae’r prosiect yn cynorthwyo teuluoedd i oresgyn yr heriau maen nhw’n eu hwynebu drwy gymorth wedi’i deilwra i bob aelod o’r teulu, cynyddu eu hyder, llesiant a gwydnwch, gan sicrhau diogelwch pob aelod o’r teulu a datblygu eu gallu i gynllunio at y dyfodol.
Gan weithio mewn partneriaeth gydag asiantaethau lleol eraill rydym ni’n dynode ystyriaethau’n ymwneud ag iechyd meddwl, cam-drin domestig a gwrthdaroteuluol yn gynnar, ac yn darparu cymorth wedi’i dargedu er mwyn atal problemau rhag dwysau.
The Families Together Project offers three types of support:

Led by a Family Support Worker, the family will receive a package of support tailored to meet their individual needs, circumstances and learning styles which builds on their strengths. Interventions will comprise of: Bydd ymyraethau’n cynnwys:

Atal y Fro will deliver a co-ordinated response (in conjunction with existing services in each locality), looking at the needs of the whole family, whilst ensuring the safety of victims. Interventions can be provided whilst victims are still in an abusive relationship: by working separately with both members of the couple, support will be given through individual and gender specific group work. Support will also be provided to ensure parents (if appropriate and safe) develop and maintain on-going The group programmes support participants to: Once both partners have attended these courses and have acknowledged the impact of abuse on the family, they will undertake couples work, with the aim to live safely together or separate amicably. Bydd Atal y Fro’n cyflenwi ymateb cydlynol (mewn cydweithrediad â gwasanaethau sydd eisoes yn bodoli ym mhob ardal), gan edrych ar anghenion y teulu cyfan, a sicrhau diogelwch dioddefwyr. The perinatal period is a key risk time for mental health, family conflict and domestic abuse. Family Action’s independently evaluated perinatal model is a highly effective intervention in this period. Support is designed to improve parents‘ emotional and mental health and reduce isolation. Importantly, the service also aims to improve the attachment between parent and baby which is crucial for the baby’s long-term social and emotional development. Yn bwysig, mae’r gwasanaeth hefyd yn ceisio gwella’r cyswllt rhwng y rhiant a’r plentyn sy’n hanfodol i ddatblygiad cymdeithasol ac emosiynol tymor hir y babi.The service supports parents who have been or are at risk of developing perinatal depression through volunteer-led peer support, offering practical and emotional home and community based help. Parents are supported from early pregnancy through to their infant’s first birthday.
Mae’r gwasanaeth yn cynorthwyo rhieni sydd wedi bod neu sydd mewn perygl o ddatblygu iselder amenedigol drwy gefnogaeth cymheiriaid gwirfoddol, gan gynnig cymorth ymarferol ac emosiynol yn y cartref ac yn y gymuned. Caiff rhieni gymorth o ddechrau beichiogrwydd tan ben-blwydd cyntaf y plenty.
