What is social prescribing?

Social prescribing is about giving you access to activities, social groups, services and advice based on your needs and interests. The aim is to improve your sense of wellbeing, which in turn can often have a positive impact on your health. It doesn’t replace the medical role your GP plays but is an addition to the service they provide.

Our Social Prescribing service is available to anyone over 18, including the parents and carers of children and young people.

Our Service Aims

In partnership with the Hackney Health and Wellbeing Board, our service to improve the health and wellbeing for adults living in City and Hackney, and reduce local inequalities in health and wellbeing outcomes, which have been amplified by the COVID-19 pandemic (Hackney Health and Wellbeing Strategy 2022-2026).

How We Work

A team of support staff and volunteers will work with GPs and other health professionals, VCS (Voluntary and Community Sector) community organisations & statutory services, to provide targeted support with a focus on identified service areas, such as financial hardship/cost of living, homelessness/temporary housing, mental health, sexual health, weight management and tobacco abuse.

Through a new targeted approach to community outreach, the service will look to better engage marginalised groups, including, but not limited to:

  • LGBTQA
  • Carers
  • Vietnamese
  • Congolese
  • Charedi
  • Turkish
  • Turkish Cypriot
  • Kurdish
  • South Asian and Afro-Caribbean residents in City and Hackney.

Alongside the existing service delivered through our Social Prescribing link workers in GP practices, support will be provided by a team of dedicated Social Prescribing Outreach Workers and Volunteers to deliver community groups, workshops, and one-to-one sessions. The sessions will define and achieve individual health and wellbeing goals and connect individuals to ongoing sources of support in their local community. Individuals will be offered practical and emotional community-based support as well as statutory services.
