Social prescribing is about giving you access to activities, social groups, services and advice based on your needs and interests. The aim is to improve your sense of wellbeing, which in turn can often have a positive impact on your health. Alongside the existing service delivered through our Social Prescribing link workers in GP practices, support will be provided by a team of dedicated Social Prescribing Outreach Workers and Volunteers to deliver community groups, workshops, and one-to-one sessions. The sessions will define and achieve individual health and wellbeing goals and connect individuals to ongoing sources of support in their local community. Individuals will be offered practical and emotional community-based support as well as statutory services.
